I agree with Keri that you should check with your doctor, but I don't think that endometriomas shrink on their own. I had kissing ovaries and mine were wrapped behind my uterus and adhered to my uterus and also to my rectum. From my understanding in researching this specific phenomenon, it is a sign of advanced disease and usually signals stage 4 endometriosis. As Keri mentioned, complications can arise, such as torsion and rupture. I had endometriomas rupture multiple times and I wouldn't wish that pain on my worst enemy. Even though surgery can feel scary and it is expensive, I am glad that I had surgery. I have seen a significant reduction in pain. I would not recommend ablation; excision is the gold standard and was the only thing that helped me get back my quality of life.
