Wife recently diagnosed with a large endometrioma and endometriosis

My 28yo wife was recently diagnosed with an 8 cm endometrioma and endometriosis. She has only seen a general obgyn who scheduled her for laparoscopic removal of the endometrioma and said that she would "clean up" the endometriosis while she's in there. Since that last visit when surgery was scheduled we have since recognized the importance of having the endometriosis removed by an experienced surgeon and particularly someone trained in minimally invasive gynecological surgery. She is symptomatic with pain and constant bleeding. We are currently living in Colorado and about to move to Oklahoma and are unsure how to proceed. Should we keep the surgery with the general obgyn in Colorado to go ahead and remove the cyst given its size? Cancel surgery, risk ovarian torsion by waiting, and seek out an experienced endometriosis surgeon? We do not have kids but would definitely like to in the future. Any thoughts or suggestions are appreciated.

  1. I'm in CO (hello!). I just moved here, but my MIGS trained surgeon recommended Dr. Ritchie Delara, who is MIGS trained. You may want to get on her waiting list for the future. From my personal experience, I know this is a very tricky decision. Since the endometrioma is so large and painful, it may be best to have the surgeon remove it now...I had a ruptured endometrioma twice and I would not wish that pain on anyone. It also meant that when my surgeon went in to do excision, the field was compromised because the ruptured cyst had left debris in my pelvic cavity.
    Then you could see Dr. Delara for follow-up and potential future surgery, if needed. You may be able to find someone closer to you in OK, but I know MIGS trained surgeons are hard to find. I wish you so much luck and reach out if you have more questions. ~Katrina, Endometriosis.net Advocate
