Chronic back pain with no answers. Please help!

Hi everyone,

I’m new here and relieved to have found this site where it feels like the pain I’ve been experiencing is valid when it has so often been dismissed.

I’ve been dealing with severe lower back pain/spasms for over 4 years now with no answers and am wondering if it could maybe be endometriosis. Since I’m young (now 25), lift weights, and have scoliosis, my pain has been routinely dismissed as being muscular or spinal. I’ve tried strengthening my back, going to the chiropractor, and doing yoga. But the flare ups I continue to experience are ongoing, have no clear trigger, and aren’t relieved by muscle relaxers or ibuprofen. I’ve been saying for years that it feels like deeper inflammation than just muscular with the sensation that my internal organs are on fire. The pain is debilitating to the point where I need to take off work, can’t do household tasks (like dishes or laundry) and sometimes even need help getting off the couch or I’ll collapse. It usually resolves after about a week.

More recently I’ve noticed that there’s a trend where I get an intense flare up about every 6 months, and it’s always at the time of month of my period. I’ve been on hormonal birth control combination pills for 10 years so I know it’s not a “real” period, but there seems to be a connection.

I guess my questions are:
A) if anyone else has experienced anything similar?
And B) if it’s possible my pain is related to my period or endometriosis even though I’m on birth control?

I have gone to my gyno about this pain and done a transvaginal ultrasound with no abnormalities, but she has referred me back to my pcp who has no answers for me either.

  1. I should also mention that I have discomfort, inflammation and pain in my back fairly often around my period every month. It’s just every 6 months or so that I’ve had flare ups to the point where the pain is debilitating.

    1. Hi and thanks for posting here! I think this definitely warrants seeing a gynecologist who has experience with endometriosis, so I would seek out a second opinion. You can definitely still have symptoms even if you are on the pill. The fire-like sensation you mention is an especially big red flag for me. There is no reason for anyone to be debilitated by pain; that is definitely not normal. Have you heard about Nancy's Nook yet? It's a great resource for finding a doctor skilled in and experienced with endo: https://nancysnookendo.com/find-a-doctor/
